“But I’ve Used It for Years”: How You Can Become Allergic to a Skincare Ingredient You’ve Always Tolerated

One of the most common things we hear from patients diagnosed with allergic contact dermatitis is: “But I’ve used that product for years.”

It seems counterintuitive. If your moisturizer, shampoo, fragrance, or sunscreen has never caused a problem before, how could you suddenly become allergic to it?

The answer lies in one of the most remarkable features of our immune system: memory.

Allergic contact dermatitis is a delayed, T-cell–mediated immune reaction. Unlike an immediate allergic reaction—such as hives developing shortly after an exposure—contact allergy develops through a process called sensitization. Many contact allergens are small chemicals called haptens. On their own, they may be too small for the immune system to recognize. After penetrating the skin, however, they can bind to proteins or otherwise modify them, creating something the immune system recognizes as foreign.

Antigen-presenting cells in the skin process these signals and activate allergen-specific T cells in the draining lymph nodes. Those T cells proliferate and differentiate into effector and memory populations. Importantly, this sensitization phase may not produce a visible rash.

In other words, your immune system can be learning to recognize an allergen while your skin still looks completely normal.

This is why previous tolerance is not proof of future tolerance. Sensitization may occur after repeated exposure over time. A product used uneventfully for months—or even years—can contain an ingredient to which the immune system eventually becomes sensitized.

Once sensitization has occurred, the biology changes.

With subsequent exposure, allergen-specific memory T cells recognize the antigen and initiate an inflammatory response much more efficiently. Cytokines and chemokines recruit and activate additional immune cells, producing the redness, swelling, scaling, and intense itching characteristic of allergic contact dermatitis.

But an especially fascinating part of this story is what happens after the rash is gone.

Some memory T cells don't simply return to the circulation. A population known as tissue-resident memory T cells, or T_RM cells, can remain within the skin. These long-lived immune cells are positioned in tissues where an antigen has previously been encountered and can respond rapidly if they see it again.

From an evolutionary standpoint, this is incredibly useful. If your skin has encountered a pathogen before, keeping immunologic sentinels at that site allows the body to respond much more rapidly the next time.

In allergic contact dermatitis, however, that same protective system can work against us. The immune system has developed a durable memory against something that is not actually dangerous.

This also helps explain an experience that can be enormously frustrating for patients: removing the allergen does not necessarily make the dermatitis disappear immediately.

Once the inflammatory response has been activated, simply removing the original trigger does not instantly reverse it. Activated T cells and inflammatory signaling persist, and the damaged epidermal barrier requires time to recover. Tissue-resident memory T cells may remain in previously affected skin even after the visible dermatitis resolves.

So there are really two different timelines: the timeline of exposure and the timeline of inflammation.

You can stop the exposure today. The immune response it initiated may take considerably longer to quiet.

And if the allergen is encountered again, those memory populations allow the immune system to mount what is essentially a recall response. The skin is no longer encountering that substance as if it were new. The immune system recognizes it.

This is why, in the contact dermatitis clinic, the sentence “But I’ve used it forever” doesn't argue against an allergy.

In some ways, it helps explain how one developed.

None of this means that every ingredient capable of causing contact allergy is inherently “bad,” or that everyone exposed to a contact allergen will become allergic. The development of allergic contact dermatitis depends on the allergen, concentration and frequency of exposure, skin-barrier integrity, and individual susceptibility, among other factors.

But it does explain why we think carefully about repeated exposure when formulating products for sensitive and allergy-prone skin.
